Position Summary:
The Patient Care Coordinator Expert will be responsible for ensuring smooth patient flow, delivering excellent customer service, and coordinating all aspects of patient care within the hospital. This individual will act as a liaison between patients, healthcare providers, and administrative teams to optimize patient experience and satisfaction.
Key Responsibilities:
Patient Coordination:
- Schedule and confirm patient appointments, follow-ups, and referrals.
- Ensure patients understand their treatment plans, including diagnostic procedures, medications, and follow-up visits.
- Address patient inquiries and concerns promptly, with empathy and professionalism.
Administrative Support:
- Maintain accurate and updated patient records in compliance with confidentiality policies.
- Coordinate with insurance companies for pre-authorizations, billing queries, and claims.
- Assist with discharge planning and post-hospitalization care instructions.
Team Collaboration:
- Act as a point of communication between patients, families, and medical staff.
- Work closely with departments such as nursing, pharmacy, and laboratory to ensure timely service delivery.
- Facilitate interdisciplinary team meetings to discuss patient care plans.
Quality Assurance:
- Monitor and evaluate patient satisfaction metrics and address areas for improvement.
- Implement initiatives to enhance patient engagement and hospital service standards.
Education & Advocacy:
- Educate patients and families on hospital services, available resources, and wellness programs.
- Advocate for patient needs to ensure comprehensive and compassionate care.
Qualifications:
Education:
- Bachelor's degree in Nursing, Health Administration, Social Work, or a related healthcare field.
- Advanced certifications in patient care coordination or case management (preferred).
Experience:
- Minimum of 2-3 years of experience in a healthcare setting, preferably in patient care coordination or customer service roles.
- Familiarity with healthcare systems, medical terminology, and electronic health records (EHR).
Skills:
- Excellent organizational and multitasking abilities.
- Strong interpersonal and communication skills, with a patient-centered approach.
- Proficiency in using hospital management software and Microsoft Office Suite.
- Problem-solving and critical thinking skills to address patient and team needs.
- Ability to work collaboratively in a fast-paced environment.

We are seeking highly competent and driven individuals to join our formidable team as General Duty Doctors . The successful candidates will be working as part of the general duties doctors in the hospital attending to patients in the clinic/wards and participating in surgeries when necessary.
Applicants must possess basic medical degree (MBBS or equivalent) and must be Fully registered with the Medical & Dental Council of Nigeria (MDCN) and with current practicing licence. Ultrasound Scan and surgical skills will be added advantage.
